Our team of IRIS Consultants help you to get enrolled in the Wisconsin IRIS Program.
What does this mean you may ask?
Well, let's start here. You or a loved one you are caring for feel like it's too hard to maintain the life that you have in your home without more help. You may be having struggles with things like:
This will lead you to the question, "Where can I find more help and what programs are available for me to use with what I'm going through?"
In IRIS, we help people in three groups:
If you are in at least one of these groups, you could be eligible for state funding through IRIS to help you live more independently in your home.
For more eligibility information follow this link: https://www.dhs.wisconsin.gov/iris/eligibility.htm
If you feel like you may be eligible, contact your local Aging and Disability Resource Center from this link:
Your local ADRC will send someone out to do a screen to see how much help is needed and help to set you a budget for IRIS funding. With this funding, you will be able to hire workers to help you with whatever problems you feel you are having.
Referral to IRIS Consulting Agency (ICA)
First Person Care Consultants or the ICA of your choice will then get a referral from the ADRC to enroll you into IRIS. Then it's our turn to help you get through the paperwork of the program. We walk with you through the state program paperwork and logistics making it easier for you to hire and train your workers, live the life you want to live, and be happy and healthy!
